Regarding the question on : To connect to On-prem we see you only need you ST-PI tools to be up to date, no installation is required, who configures ALM to talk to the On-prem system?" : refer to SAP Community blog First steps to work with SAP Cloud ALM Deployment scenario for SAP ABAP systems (7.40 or higher) 

There are additional configuration required to work with the Managed systems in Cloud ALM.
