Hello everyone,

I am trying to make a report where it shows if people have certain codes. It is doing what I want but it is showing the person's name twice or three times. I also try different IF functions, but the name will come up multiple times. Here a example how I potentially want it. 

Employee           Life half | Life Ins | LTD sup | LTD

Jacob                  40                          42 

Bob                                41                           43 

I am not sure if there is a way to get the system to do the example above. Could there be a way to make different queries then merge them and they I can get the name once will all the codes I want?
