approval does not trigger provision anymore

Hi folks... getting crazy with this IDM stuff >_<

After solving my last problem (see https://answers.sap.com/questions/13143019/idm-approval-issue.html) I did some further testing and debugging which led me to create a new incident at SAP, I tried to test the approval thing again for the documentation.. now, approvals do not trigger the provision anymore..

The privilege is requested -> ToDo tab of the approver -> approval is made -> nothing is happening, the status remains in "to be processed" but in the details it's stated that the approval is already made, on db level I can see an open pending value opject with execstate=1. It should trigger PlugIn 4 AssignUserMembership but it doesnt.
PRIV:ONLY is assingned, PRIV:SYSTEM, too. USer Exists in IDM and backend. No further open changes for this MSKEY.

I don't get it.. Maybe I am not seeing the wood for the trees... please help
Using the latest IDM 8.0 patches..
