To fulfill your requirement, You need to make use the Template.
I m explaining you the header part i.e.,
To Achieve first 2 column, you need create 2 rows and 2 columns as shown below in marked box.
To achieve next 2 two columns, you can use
First row 3rd column, width is 8cm
In 2nd row , divided it into 4 parts as shown below assign the value.
I think you maight be knowing how to assign the values to Template.
