selection screen

while developing a report program under At Selction-Screen on field,i have a perform statement,when i double click on it will ask for creation,but even after i created and after executing the report, when i want to go to the form through perform it is again asking for creation of the form even though it is already been created, can any body tell why is it happening?

utilities -> update navigation index
