Return Request Process Automation Issues

Hi Experts,

I am trying 2 cases but can't run the template properly, 

Cases 1: Add template from Store, the template in status releases and I am able to be deployed it, but when I start new Instance it's not send to my in box to approve document, and process status is running as below

Case 2: Create from template, I am able to update SDK & Agent Version to latest, but when I run test for Automation it's said that Step name [XXXXXX] already used by another step.

Please help to share your ideas or experiences to fix these issues above.
