I'm using transaction MLS5 / program LMLSTF36
(Standard Service Catalog - Import File).
I created an input file based on Note 559688, but it's rejected. A working example will be helpful.

I guess this answer is coming WAY too late for you buddy. I was having trouble with the same transaction a few weeks ago and I couldn't find any example to make it work. That's the reason why I am answering anyway to help out people with MLS5.
Attached to my answer is a working sample. It refers to a Ramp Services Catalog.
