What could cause this to happen. We have been seeing this occur very randomly and cannot figure out what could be causing it.
Any ideas or thoughts would be appreciated as I'm lost now.

It looks like your transaction log is possibly corrupted - the best option is to go to your backup.
Can you translate the log successfully using the Log Translation utility (dbtran)?
