1. You can always use the Fiori login page instead of QR code.
2. If you are talking about access Fiori Mobile app, then give the access of only apps which the user will be using to approve, reject in his work flow and necessary access require to process further the action in background.
3. It will depend upon your Mobile or Windows setting, how much time it keeps the session activated.
