Hi,
have you tried comparing (textually) content of your model with some previous version, which has correct formatting? Has it really changed? What actions took place in your model after "formatting ok" and before "formatting on defaults" Do other users have access to your model? Do all of them use exactly the same PD version? Could anyone by accident call "Default" function in Display Preferences?
Model type, PD version, more detailed description of the situation, perhaps screenshots and/or sample models. This is the list of things which are often missing in your questions a which are preventing you from getting better answers/help.
