[BUG] Former Member and Name shown

In the list, a question is asked by a "Former Member" and at the same time has been published by a named user:

When going to the question page, again this information is shown inconsistently:

Hello,

It is two separate user accounts. Original Author of the content is blocked. Answer was published i guess from the same person, but another user account.
And as vadim.kalinin mentioned some moderator activity resulted 'published' record.
