Can anyone tell , how to make smartform for MSDS , and what is the table name for the data in property tree - like Hazardous Identification , table for value assignment,

nobody in the EHS world is using smartform to generate a SDS/MSDS. Anybody (who i know) is using WWI technique only

even if you suceed to may be generate SDS/MSDS using Smartform => there is no real "value" ýou will add using this approach.
The whole SDS/MSDS dispatchment is designed to work only ! with WWI documents (and documents of type "inbound" (e.g. pdf).
What is the background of the business demand to use Smartforms?
To get some idea how EHS is "designed" e.g. check the existing output variant. Here you can learn how SAP has designed the approach to retrieve data from database and to present the result to the user.

you will find as well all the table names needed. Or if you would like to start from "ABAp" point of view: check SAP packages CBUI. Here more or less thé basic EHS stuff is managed. As explained. For EHS purposes (retrive data): e.g. take a look at:

The most important APIs are part of function group C1F5 (new ones) and C1F2 (old ones)
I assume and hope that you should have now the "core input" to look for solutions

As well you will find on your SAP system reports as:
RC1_1077_BAPI_EXAMPLE*
These are examples of reports delivered by SAP. They try to show the correct use of the core BAPIs (and they are a "must to read and understand" reports to e.g. read, or write data to EHS database

If you have no clue about EHS data structure and APIs, BAPIs to use to retrieve data from EHS data model stop your action. It is not worth the work. Without using EHS at least for 3 years you will not be successfull.
Why do you not use WWI ? This is the right tool to generate SDS/MSDS? As mentioned. Nobody in SAP world will use SmartForms (or similar solutions)
Even by providing to you the table names used in EHS: It will not help you. You must have a good (very good) understanding regarding the design of EHS data model. This is different in comparison to e.g. SAP MM etc.
