Capture quarterly rating on PMGM

In our company, we plan to put in place a quarterly review (rating) for the performance management process. The employee will be rated every quarter and there will be a final evaluation at the end of the year. Is there anyone who implemted a multiple rating process during the year? How did you proceed? Single form or multiple forms?
