Need clarification on AppGyver

We are from WBC Software Lab Pvt Ltd,We are trying to do test with App Gyver application. But we are not able to access https://<sid>:40000/ServiceLayerController/

Always showing this empty screen while running this URL.
Is App Gyver only used in SAP B1 Cloud?
We are using SAP B1 9.3 PL04 on permises.
Kindly guide us to rectify this issue.

Dear Experts,

While trying to connect load balancer, Its asking user name and password... Which credentials we have use over here. Either DB credentials nor Site User credentials, We have tried both but its not getting connected. Please refer the screen shot.

Try this in the user name {"CompanyDB": "YOURDATABASE","UserName": "YOURUSERSAPB1"} , In password type your "password".
