It is not standard, it varies for each Organization where we can give functional logic such as Tables, Fields,calculation method,Formula's etc.

FS Documents: - Functional specification is the document, where function consultant provides the details information i.e. technical logic (through Field & table or through Functional Module or other way) how to fetch the data in which field or which table…. & how to display, as per the requirement by the end user of clients.
There are no specific or standard document of FS depends upon company to company or consultant to consultant etc.
