how can we display this feilds in report ,i think like we cant use alv_grid_display ,or alv_list display can be used.
can i have a sample code or refence for developing this kind of report.

Have you checked the code of the report you are showing in your print-screen?
A lot of this can be done with write-statements. And I guess that probably some events like TOP-OF-PAGE, TOP-OF-PAGE, etc might be involved as well.
