Can anyone please advise what more steps to be taken in case of production system DB upgrade from 11g to 12c where oracle data guard is also configured. What activities we need to perform on standby host so that the DR site is also upgraded with the primary site ?

The main steps are mentioned in link:
https://docs.oracle.com/database/121/SBYDB/upgrades.htm#SBYDB1200
Then you need to refer to SAP Oracle upgrade guide to do the "upgrade phase".

The installation of the Oracle 12c database software is an online operation and you do not need the databases to be shutdown for the software installation. This is done on a new filesystem. You install Oracle on primary and standby databases and apply the latest SBP on primary and standby. You perform the pre-upgrade tasks on primary. You will run the the upgrade on the primary database host as per the upgrade guide and during this time you will ship the logs to the standby host to get the standby database upgraded as well.
