How to generate a cuctom code report

I urgently need a  Custom Code report for the s4hana upgrade projects, where and how do i generate it on Solution Manager or using the Scope and efford analysis?

There are already reports available for some systems on https://me.sap.com/app/customcodeanalytics.
Regarding the content in report, in case further interpretation needed, please press button "Service Request" on top right corner of the report.
