Custom Form is not triggering through Create Correspondence Fiori App

Hi,

I'm able to trigger the Standard adobe form of Account Statement (SAP06) through 'Create Correspondence' App in Fiori Launchpad but getting an error when triggering the Custom Form as below.

I have created that Custom Form in 'Maintain Form Templates' App and maintained that in Assign Form Templates in SPRO.

I have followed all the steps mentioned in the blog -> Output Management and In-App Extension for Account Statement forms in SAP S/4 HANA | SAP Blogs

Can someone help me out in this.
