Add logo and custom data to MM_PUR_PURCHASE_ORDER Form

Hello Experts,

I am wondring how to achieve to add a custom logo to this form ( Multiple logos depending on company id) and want to add other informations data.

For the logo I did the following :

-Add new entity while redefining Odata service "FDP_EF_PURCHASE_ORDER_SRV":

-Implement the GetEntity related to the new added entity

-In the app " Maintain Form templates" , I copied the Standard Form to Z one and I did the following :

- I used this master form :

- I Download The form ( With Master form included ) .

- Import xdp file to Adobe live cycle, but I don't find the created entity :

Please tell me If I am missing something ?

I don't see app Maintain Form Templates, how can I get it?
What role do I need?
