we had a similar case in an EWM S42019, so I am not sure whether my hint will help.
Our issue was on the ERP side (S42019) which did not communicate the hierachy correctly. We were able to see the error when debugging on the ERP side. Eventually we raised an OSS message with SAP and they sent us a preliminary fix (still ongoing).
Hope that at least gives you an idea where to look next.
