APO cannot be "migrated" (technically) to S4H and IBP. Both are products / solutions where you have to decide how you bring your current APO processes and solutions to these 2 applications.
First of all - SAP provides APO integration for S4H too (like they do for ECC with the plugins) - you can still use the most prominent APO modules DP,SNP,GATP if you switch from ECC to S4H. Check the SAP Portal for more details which S4H version still allows usage of which module (for example EWM & SPP in SCM/APO will probably not be supported with S4H anymore). How long SAP will do this (until which S4H Version) is not known at this point.
Second: check the corresponding solutions in S4H and IBP to replace your APO solutions. Examples: replacing APO-GATP could be an S4H question if the current state of AATP ("Advanced-ATP" in the SD module) would be a path. Other example: IBP comes with a "DP" like and "SNP" like module again and your DP and SNP processes could be setup in IBP again.
Third: check the conversion service SAP has recently announced in their IBP WebCast - they start offering a service offering for APO customer to ensure the transition from APO to IBP is smooth.
