WI completed without 'Wait' Step Event to get trigger

Hi Experts,

I have some issues on production server where WI is getting completed without executing the 'Wait' Step to trigger. This event for Wait step is trigger when user takes any decision from Outlook. Inbound Mail is configured in SAP which is responsible to get the Response and update the Shopping Cart with with response. Issue is for some action taken (Decision taken Approve/Reject) on outlook WF behaves fine and for some WI gets completed but 'WAIT STEP' is not executed. Graphical view shows that WF still waiting for the WAIT event to be executed but WI gets completed as shown below.

Appreciate your help on this issue.
