SAP SRM - Worklist Items incorrect status

Hi, we have recently upgraded to ECC EHP8 and netweaver 7 and SRM EHP4

Since then we have had a few occurrences where shopping carts are created, approved, confirmed and fully invoiced yet the Line item status on a number of lines have "In Purchaser's worklist" whereas others have the correct item status. I cannot see why this is happening and because it doesn't happen all the time I cannot reproduce this.

If I run BBP_get_Status for the Shopping Cart the 1 line status correctly then reverts back to "In Purchaser's worklist"

Any ideas?

Does the SC have correct status, when you execute it directly for the SC?

Do you have any example that still has the incorrect status? If yes, please share the SC quantity and the item field QUAN_PO_E.

Moreover please provide the SP level of SRM.
