PO Header status in awaiting approval

Hi Experts,

We have an issue with the below scenario

When the PO with ordered status is changed and then the workflow(process controlled) will be re triggered and all the agents gets the work item, when all the agents approve the document the header status still remains as 'Awaiting approval' where as in the item level it shows as approved by all agents workflow log

Shows no error and it is completed successfully.

The issue is with the changed version of the document.

Can anyone help me on this issue
