Custom Category in Notes and Attachements Tab in Shopping cart Item detail

Hi Friends,

I have one requirement,

In Shopping cart Item Details there is one tab NOTES and ATTACHMENT tab, there are two standard categories

1. Supplier Text

2. Internal Note

now i want to add one more category i e

3. Item Category ( Custom one )

can any one please help me how to get this
