ANy help on customizing the Questionnaire in SUS-Self Registration in SRM7

Can any help me on how to customize the Questionnaire in Supplier Self Registartion? Customer is asking wthether we can have the file upload facility in the questionnaire against some questions? Or if it is not possible can we send the questionnaire in a mail and ask the bidder to upload the necessary documents to it in a return mail? I really appreaciate, if some body could help.
