if you follow the tag where you answered the question then just switch to the Activity tab and you will see that the user performed the action "reject" for an accepted answer. You are not notified about negative events.
User could have changed his mind and may accept the answer at a later time, after he has implemented and tested what was in the answer.
Nevertheless I personally think that there is something wrong as the same thing happens to me more often as you can imagine.
I have some theories:
- user performs double click where the first accepts and the second rejects the acception
- platform is extremely slow and the accept is not shown immediately , user clicks again, then the result of the first click is shown and user leaves page and does not realize that the second click had rejected the acception .
