Is it possible to follow discussions across the communities by specifying the keywords?
Example:
I want to have mails / Activity alerts, everytime some discussion is posted with a title having a word LSMW.

You can filter through recent content containing a keyword. Browse > Content > [filter by text]. e.g. http://scn.sap.com/content?query=lsmw
Offsite, you can use Google Alerts: Google Alerts - Monitor the Web for interesting new content
Just prefix your query with site:scn.sap.com to see results only from SCN according to a schedule you define. e.g. site:scn.sap.com lsmw

Tags are the closest thing to what you are looking for.
Every content tagged with lsmw can be seen by browsing the tag.

But you are looking for a way to get notified. Since we can't follow a tag, rss feeds could be the next best thing.
This is the lsmw feed URL that you can check out. It can be added in any rss aggregator. My personal favorite (after demise of Google Reader) is Feedly.
