It seems that some questions show up twice in the list. I've seen this a few times before but didn't pay much attention. Screenshot is below, it's from a random visit to the global Q&A list (sorry to put OP on the spot!).
As you can see, this shows 2 items that are exactly the same. Question has no edits that I can see (also it says "asked" both times). I've opened the first link and when I got back both items changed color to one of the 50 shades of grey.
Can it be that there was a click on the famous More button before this happened?
At least this is my experience. I never saw it on the first page directly.
My impression is that after a click on the more button the system starts again reading from the most recent posts (and there have been some new posts since I accessed the page), it then does not add the new posts on top, but it somehow shifts the last post of the current page this number of new posts down the stream and duplicates it this way.

Thanks, Jerry. This happened again few days ago and it seems Juergen was right - it's tied to the pagination. It seems to occur when I have the list open long enough for additional questions to be posted, which seems to throw off post/page counter.
Well, this is just one more thing in favor of endless scrolling...
