ankit.chauhan1 posted an answer to my question, and he commented on that answer, but the comment event cannot be found in my inbox. He mentioned me in both the answer and comment, and only those events show:
Only things special that I can see, is that he is a moderator and SAP employee.
I also did not receive the customary email notification about the comment:

1. You have received 1 email for the Answer Ankit has submitted.
2. You have received a notification in your Profile Inbox for the Answer Ankit has submitted, and you've set it to "read" (or basically clicked on it to show more details which also sets it to "read".
3. You have also received, as you also mentioned above the mention notifications. Those were sent only to your Profile Inbox.
The reason you haven't received the notification for Ankit's comment (except for the mention) is because Ankit has commented on his own answer. With the current configuration of the notification rules in order to receive that notification as well - you need to follow your own question - this way you will receive all updates on all levels of the threads under your question.
We are looking into how to improve the notification rule for better user experience. We'll post an update once that is done.

An idea on how notifications should work for an optimal user experience, was suggested already back in 2016. SAP started planning this feature, and did exactly as I feared, they patted themselves on the back, and then forgot about the rest of the plan apparently: "We are looking into how to improve the notification rule for better user experience." So, look no further, just look at the original plan and/or the idea mentioned above.

Does this mean, that if I explicitly follow a question, I get notifications of ALL activity in that question, independent of who asked it, who answered it, and who commented on either question or answer?
If so, I would say you have already given yourself the answer: "...you need to follow your own question..." How about simply setting a question automatically to "Followed", upon any active interaction with it (i.e. ask it, answer it, comment on the question or an answer)?
In the (I expect extremely unlikely) case someone does not want to be informed of all activity, they can simply explicitly "unfollow" the question.

Hi, Johan,
Indeed when you follow a question or a blog, you will receive notifications for ALL activity in that content item (Question / Blog).
So in that your suggestion you have linked above is fulfilled. Also whenever you receive a new notification you have an orange indicator dot on your avatar picture in top right corner, so that's a call for action to check the new notifications - that answers another part of your suggestion.
In regards to automatic following your own content - that's an option that is still on the table. But there are several ways of doing it with its pros and cons. This is being evaluated.
