I checked the issues for following a question and a blog post. First one is currently in progress and being worked on.
Second issue affects migrated blog posts, i.e. if you are following a blog post which has been migrated from current SCN to the new landscape. We are following up on that.
As Sajid is pointing out, activity stream lists updates if you are following a person, blogs and primary tags. So if a user is following a primary tag, e.g. "ABAP Development" and another person is following a post which is tagged with that primary tag, then it will appear in the users activity stream

I see two issues that you are pointing out:
- Why the content link is missing
- Why certain activities are showing up on your activity feed
To answer your first question, could you please explain if all other activities are showing the same issue? That is are all activities with content in your activity feed not linked?
As for the second issue, three types of things can show up on one's Activity feed. And these are specific pieces of content, people, and primary tags. One would have to follow any of these elements for them to show up on one's Activity Stream. Perhaps you are following the blog that Fredetique DUFY started following. However, I am not sure if this action should show up on your Activity Stream. I am looping in to shed some light.

Other activities usually have the link to the person, the question, the blog followed.
I just checked. Only Frederique DUFY's activity is still like in the screenshot in my activities.
I guess that this blog was rejected and is not visible to me. But if this is the case then it should not be listed at all in the activities.
I hope Mirko can shed some light why I am getting Frederique DUFY into my activities.
