In fact good question.
without knowing the difference, will be difficult to deal with the SAP EH&S - especially product safety & DG.
specification is at high level and it categorized in to -
Substance
Agent
Packaging
waste code
DG classification
you can find this same structure in the stransaction CG02BD.
under the substance - again can be diffirentiated as Real substance, Listed substance & Pure substance etc.,

finally we can say in this way.
All substances are specifications
But not all specifications are substances.
If you take this in organization point of view (out of sap), the products will be called as substances and the hazards (agents) like noise, dust, temperature, etc., are considered as specifications not as substances.

Based on SAP EH&S Core customizing and EH&S core functionality the selection options etc. of CG02BD changes (e.g. which search functionality is availble; which user exits can be used etc.).
The term which SAP uses is:
Substance, Agent, Packaging, Waste code, Dangerous goods classification

If you would use on the top
SAP Recipe Management
these categories are available as well:
Recipe, Formula, Equipment requirement, Process element
Regarding The "Real substance, Listed substance & Pure substance etc." . These are known as: Specification types
