I try to create and add scoring on item level but i cant because scoring model not active on item level

Not sure if SAP provides 'Scoring model' tab under Miscellaneous view, like Questionnaire.
Check the first tab 'Overview' or 'Metrics' under Miscellaneous view and try to find that field 'Expected Commercial Value'. Because, your scoring model is assigned to those fields.
If you do not find that field nowhere in Item, then you can configure same field via "Assign Fields to Views/Subviews" customization.

Sit calmly and re-check your configuration properly.
I can see, you have assigned your Questionnaire to "PROBC - Probability of Commercial Success" in field configuration customization but same PROBC is not appearing as Field under Miscellaneous tab.
Remember, you cannot assign Scoring model and Questionnaire to a same field.
A Questionnaire of field (e.g. RISKA, PROBC) helps in finding scoring for other field (e.g. PROBT).
Hope you can correlate now.

If you are not able to update field directly in scoring models configuration, then maintain 'RISKA' and Scoring Model service in Field configuration.

Please share scoring model configuration called as "Define Scoring Models".
Cross-check once there whether the same scoring model assigned to object type 'RIH - Item' and 'PROBT - Probability of Technical Success'.
I do not have system access otherwise I would have sent you steps by steps config.

To achieve that, please visit to customization "Define Services" and maintain 'Linking object types to service' for linking Item to Scoring Model.
Also, maintain or ensure that same "Scoring Model" have been maintained in Services (do not remember exactly name) tab in Bucket.

depending on the field configuration, either Questionnaires OR Scoring models can be selected in a bucket.
The customizing for field configuration can be found as follows:
SPRO
> Portfolio Management
> Global Field Settings
> Check SAP Field Configuration
for example field PROBT (Probability Technical Success) is linked to service 'Questionnaire' and not 'Scoring model' so you can only select a questionnaire for this service.
Regarding 'What-Ifs': they can be only created for 'Reviews' not items! Open a review and go to tab 'What-If', press button 'Create' and then you can select one of the scenarios types defined in your attachment.
