SAP S/4HANA EHS Incident Management issue: task created several times.

Dear Community I seek for your help

In SAP S/4HAHA for EHS, Incident Management, OnPrem release 2023, sometimes when creating tasks the app generates am additional line with the exact same task, sometime more. See pic below:

My technical knowledge is rather limited, and I need help to direct an ABAP or "Fiori" consultant towards the right direction. What might be the cause of this behavior?

Note: either repeated task you click through will lead to the same screen and the following behaviors are exactly the same, as in when you close the task, all the repeated lines will show you that - it's just that, repeated lines.

Hi, have you had a look at SAP Note 2897861 - Fix for second task instance created for non-recurring task definition - SAP for Me

Its a note released in 2020 but may be useful to check it.

You haven't specified but is this a new implementation or upgrade of S/4HANA?

Also have you checked in transaction SWIA whether the workflow generates two workflows for the task when it is created?  I would also have a look just for in case.
