SAP Management of Change - tables where answers to questions are stored

Greetings,

we are using SAP change requests from SAP MOC component as part of SAP EHS (environment health and safety). Based on the questions and answers to these questions we have prepared follow up activities which trigger additional objects creation in S4 system.

We are struggling to find in which tables SAP stores answers to questions which users submit via change request.

Example of a questionnaire with three questions is shown in picture below.

I have checked table /IAM/D_ACT_ROOT and there are some answers but it seems there are only for question type tri-state.

Step 1 : I_ChgMgmtRequest (/iam/d_i_root)

, we will the change request ID and get the UUID.

Step2 : I_ChgMgmtActivity(/iam/d_act_root): Pass the UUID.

Step3 : Pass the Activity template (RSK, PST-1 etc in the field act_template(table) ChgMgmtActyTemplate(CDS view).

Step4 : Pass the activity UUID into I_ChgMgmtActyDescription and get the code subgroup.

Step 5 : 

pass it to /IAM/C_CODE_T for code text

/IAM/C_CODEGR_T for Code group text.

For step 3 we need to take the help from technical team since its a CDS view.

Greetings, I have checked the proposed table and it doesn't include the answers to questions. There are some values but it appears only for tri-state question type. I have added addition pictures in the original post for better visualizations of questions submitted today and SE16N display on the same date.
