Hi All
i was able to do risk assessment rating with a questionnaire method .
Please find below the steps followed:-
1. Created a new catalog group under question catalog ("02" industrial hygiene and safety)
2. Created catalog entries under the new catalog group
3. Created a Questionnaire using the newly created catalog group.
4. Assigned the questionnaire to the work area
5. Filled the Questionnaire, checked for inconsistencies and closed the questionnaire
6. In Exposure Profile, choose the questionnaire tab and did the rating by clicking on the rating button.
In SPRO ( prior to assignment of Questionnaire to Work Area) i checked the manage user exist for ANLS_QSRAT with FM -CBIH_LB33_UE_RATING_COMP and in setup analysis method questionnaire i maintained the following

The steps you followed, looks good. Here our final intention is to capture the results in risk assessment and come out with the rating. for that, you must consider the customizing settings too.
for your questions.......
1. The logic behind the execution of rating in risk assessment
As per SAP standard, we have 3 ways of analysis methods. 1. meas. value/ ref. value 2. standard. criteria 3. Questionnnaire. It can be enhanced further, depend on the business requirement. but this will be fit for most of the requirements.
Each Method (Tab) have the "Rating" Swithch, which will calculate the result based on the entries.
And in Risk assessment Main screen, "Main Rating" switch which considers the above 3 different analysis methods, and come up with the final rating.
2. If weight-age is given to questions, how rating is analysed?
check the customizing settings
SPRO > Environment, Health and Safety > Industrial Hygiene and Safety > Risk Assessment > Analysis methods > Set up Analysis method questionnnaire.
Here, Agent wise you can maintain the ratings. Based on the answers (entries), these will be calculated the ratings.
weight-age, states the importance of that question. how important that is?
for ex - IN the work area (particular site), the employees are wearing proper PPE??
this is very normal question, which is not really impact your analysis. at same time this should not be neglected. for these kind of quesiotns weight-age will keep les and for specific questions, which could impact the assessment - may have more weight-age.
3. Meaning of the statement -
As I mentioned above, the general questoinal we are repeating for different work areas.
check the above mentioend customizing setting - where you can differentiate the questions Agent wise, catalog and group wise.
Group wise, Risk rating can be defined.
Note - If you would like to have correct ratings in risk assessment, maintain the groups correctly. so that it will be possible to maintain the correct customizing. prefer to do this customizing after catalogs and groups were set in perfect manner.
4. Moreover, When and how often the questionnaire method is used in industries?
Questionnaire usually used in industry for site inspection purposes. these site inspections will be planned quarterly, monthly or 15 days.....based on the importance of the locations.
for ex - dealing with hazards like High temperatures (reactors with high temperatures), exo-thermic checmical reactions, need more attention than the other locations like administration buildings and ware houses.
after multiple visits/ assessments, you will come to know....what are do's and dont's in that particular location.
these all things will be captured as a questionnaire. each question have some weight-age based on the impact (the weight-ages will be defined in safety meetings and in shop floor meetings based on the intensity, and the joint effort between production and safety departments).

a pre-requisite for the questionnaires to work with the analysis method is, that you assign question groups to exposure types. This is done in SPRO. If you group your questions in a way that certainb groups belong to certain expsoure types, only the relevant questions will be displayed when opening the analysis method.
Regarding your questions:
1: the logic is, that the system calculates a quotient of Yes-answers to No-answers. This comes from an intrisic logic for medical questionnaires which have the bad case always when you have to answer with YES. Thus, if your answers are set up differently, you would have to exchange the logic in the user exit which is called when pressing the calculate button.
2: the weighing is used to prioritize answers manually. If you set a weighing, this factor is used (instead of '1') for calculation of the quotient mentioned above. No weighing means a weighing of 1. With the weighing, an expert can put a focus on one or more answers where he decides that they are more (or less, depending on the factor) relevant than others.
3: meaning of the statement. The qustionnaire result can be derived semi-automatically using the quotient and the weighing mentioned in question 1 and 2. Besides, for everyone who does not like such a calculation, a manual anser is always possible. In such cases it might be useful to maintain a statement, decribing why the expert set this rating and no other. That's all.
4: Questionnaires are very often used as analysis method in the industries. However, this SAP analysis method is very seldom used. I guess this is becasue the effort of setting it up is quite high and the questionanaire tool coming from occupational health is not a lighthouse in usability....
Sometimes customers create own analysis methods (up to 10 custom specific methods are foreseen) and design them in foprm of questionnaires.

I have set the analysis method for Questionnaire that a N = HIGH Risk, as in non-medical environment a N means it is not done and is a risk. A Y = LOW Risk, as a yes, means it was completed and it is good. The automatic calculation of risk should then count the entries in the questionnaire and determine the majority as Y = Low risk.
is that right?
or is the user exit saying Y=High risk (as in medical terms)?
