Hi Folks,
Just wanted to know if any one has come across this kind of a requirement,' Root Cause Analysis(RCA)' report in IHS, There is Risk Assesment & Measurement mgmt in IHS, But i haven't came across RCA in IHS. Please let me know if there a std feature or anyone has customized this kind of a report????

There is indeed standard facility of analyzing the root causes of an incident / accident. This can be done in the "investigation" tab of the incident / accident log.
Here you have a defined "cause heirarchy" where relationships between the causes are illustrated. A cause hierarchy is a tree structure that contains a series of primary cause categories (such as technical factors or personnel factors) on which other cause categories and causes depend hierarchically and with an increasing level of detail. This is even configurable as per client needs.
For a cause analysis, you can use the integration option with the IH&S safety measure object. This means you can assign a safety measure to a cause and display it in the safety measure screen.
As far as RCA report is concerned, you can use the "investigation result" field long-text facility to record unlimited character information. Alternatively, you can use "Go to --> User Defined texts and document" to upload already created documents in SAP DMS.

Hi Nishanth, The Root cause Analysis itself has got to do more out side of sap, means You should have your own methodology for RCA, Generally any RCA ultimately publish a Report. So sap has given flexibility to attach any kind of final RCA report only. as mentioned by Pavan it can only capture the hierarchy, but the process should ideally be out side of sap.
