FPM Start Event was Cancelled.

When the user was trying to open an incident in EHS portal, he is getting an error "FPM start event was cancelled".

I am not able to find where the issue was. This is happening only for one incident and all the others are getting opened without any error.

Please help me in providing some useful information as from where should I start my analysis.

I have attached the error message.

P.S. I am new to EHS portal.
