Hi Rosina,
Yes, you have missed the step to create the questionnaire.
1. Ensure the cataloggroup and catalog available - Transaction code CBIH51
2. create a questionnaire in the transaction code - CBIH52
3. Have to activate the question, by clicking on the status button. so that the question released from inprocess status.
4. to complete the above step, have to enter the catalog entry. EDIT > Insert catalog entry.
Now your questions are ready to use.
the same questions can be used in work area (CBIH02), by using new questionnaire button.
by following the above process, you will be succeed in adding the questionnaire to work area.

Hi Kamal,
I checked the process you suggested, but I'm still having the same problem, I checked for a note and found note
| 1167376 | Texts not displayed in questionnaire |
I will apply this and let you know the results.
Thank you
Regards.

Hi Rosina,
I found more relevant SAP Note for your issue. 974888
even though you create the questionnaire in the system, those will not be appeared in workarea - if you go for F4 selection.
While creating the questionnnaire in the transaction - EHSQUESTOH, Assignment category - VOR, picking up as default. bu the value should be WAH for work area.
If you could implement the note, you may get the positive result.
