Add the New form Name in sap EHS CDO

Hi Team,

I have created the new Adobe form(ZTEST_FRM) and used the respective data provider class in SPRO configuration.

Can i know the BOBF table name to add the new entry so that it will be available in below screen shot.

Can i know is there an alternate way to add so that it flows in above screen shot .
