Variable Pay Bonus Assignment Statement

Dear all,
I'm sure you can help me solve the following two points:

1. We need to configure a Bonus Allocation statement. It must be very simple: company logo, employee's name, text and footer as image. Trying to start from Personal Statement, the preview doesn't work. i'm pretty sure that the problem is the format 🙂 but searching into the web, i didn't find any example of Bonus Assignment file.
2. The client required us to review the statement by the head of the company (and sign it) before to publish into the people pofile.
