Time Off Notification

I have set up a time off notification via workflows.

The business rule is attached. The intention with the business rule is that if any of the time types below have been used 4 times or more, an email is triggered to the manager.
When the rule was activated in production, the email was sent out to manager when the time type had not been used four times and in addition it was sent out to managers regarding time types that was registered back in October 2022.

Do anyone have experience with such rules and can help out on how to get this rule consistent?
All help much appreciated.
