I created a Document template by means of the following:
1. Made a duplicate of the Training Document.
2. Renamed it.
3. Linked it to a Template that was imported under Adaptable Resources.
The template consist of logos, change history table and Table of contents. The numbering and the display of the output when generated looks clear and good, but it seemingly picks up an entire paragraph as a title. I have tested a few other options int he document settings for the template, and the numbering is set to Global All objects, but the last number header shows the entire paragraph as header. The text and formatting styles in the MS document is the same. It appears normally from the PC in the template, but once the output is generated, it changes these fonts and paragraphs. The last one.
6.1 Should not reflect in the TOC but also not in massive bold in the paragraph:

It is likely that one of three issues is causing your problem:
1. You have applied manual formatting to the text that is not appearing correctly when generated as documentation. If you have applied manual formatting to text in Enable Now, even if you have also applied a text style, the manual formatting is carried over into the documentation. To remove the manual formatting, open the text in Producer that is not generating correct, select the text and click the following button
2. You have applied the incorrect text style to the text that is not formatted correctly. So, open the text in Producer and apply the correct text style to the text
3. When you define a set of Documentation Settings - Resources > Documentation Settings - in Producer, you specify the Text Styles object that is to apply to the documentation when it is generated. Information like font size is defined in that Text Styles object. After generating the documentation, click on the text that is not formatted the way that you would like and check the text style that has been applied to the text. Then check the Text Styles object in Producer - Resources > Adaptable Resources > ... - to determine the formatting that should be applied to the text.
It may be necessary to duplicate the existing Text Styles object in Resources > Adaptable Resources to create a new object and then apply different styling to the text styles in that object. You can then apply the new Text Styles object to the Documentation Settings object to ensure that the text is generated with the correct text styles.

Thank you for your input and patience.
You shared an informative video with me recently with more detail on the Document Settings and then, whilst I was busy with that, my colleague created this word template, and the file is now corrupt it seems. I think it's because it was copied form a Training Doc SEN template, downloaded and then created manually. Following your guidance, I think it best that I just recreate it properly from within the Tool. I'll keep you posted. Thank you very much.

Ok, I have stepped through your post and herewith some clarities -
After the error, there is a break that is correct, as it was set up in the Fragments section
The error comes up regardless of the Text Styles as the content in writing pulls in from the MS Office template
