Same question is displayed as twice with different results in Survey report in SF LMS

Hello All,

Did anyone face such issues in the past & have some suggestions?

When Admins are running 'Survey report' in Successfactors Learning for Non ILT items, sometimes the same Questions are coming multiple times with different Rating bars. This is not happening with all Surveys/Items.
But there is no unusual thing in affected Survey/items. Attaching image for reference.

It is very confusing for the Admins. Does anyone know how the Survey report works, we couldn't found any KBA related to it or is this issue in the report?
