We have noticed that course prerequisites are being ignored when assigned within a program. SuccessFactors says this is working as designed, but seems like a huge system flaw because we have learners signing up for courses out of order. Has anyone come up with an alternate solution for this?

It sounds like curricula may be better path to take for your scenarios instead of the more simplistic program entity.

I agree, it is a system flaw. Approvals are also not respected within programs either.
As a workaround you could also have the pre-requisite course within the program with a restriction on the sections, so they cant get to the section containing the course B until they have completed the section containing the course A.
Or you could have the item as a link or text-type activity in the program where they have to assign the item to themselves outside of the program, meaning they have to satisfy the pre-requisite. This may mess up your reports though as the record of completion within the program status report will just be the self-certify one.
I would be interested to hear how others manage this too.

Agreed - we have used section restrictions before, but the issue is with the way we create our programs. We allow users to work in sections simultaneously because the topics are not dependent on each other, but within a section they are. We may have to rethink our approach entirely.
