Has anyone seen this issue where we are giving these permissions to the Managers but they still can see all the positions in the position org chart?

It doesn't look wrong to be fair, but as part of this KB (2437558 - How to restrict Position Org Chart on Target Population - Employee Central (sap.com)) there is 1 very small note you may want to consider:
They probably have access via another role to (some of that) info.
