Knowledge share for Performance and Goals - Performance Forms or Not?

Hi all

We are looking to redesign our Performance Process. We currently have just a performance form with a rating at the end of the year which we use for a bonus calculation. We don't currently use competencies the ratings are all based on a 5 point rating scale from objectives.

I would love to hear what other people are currently doing as a knowledge share? If you would be happy to share screenshots that would be most helpful. Bonus points for anyone who is happy to share what they do from a Pharmaceutical company.

Do you have any custom elements that have been well received? For example I have see an example of a kind of succession/talent rating as a custom section.

What is your company doing that has been really well received?
