Our administrators are looking at recording the reason why people have received failure status for classes (i.e. adding a reason field when they record learning completions history). They don't want to record using the comment field because it's free text and everyone will have a different wording for the reason, which makes it impossible to report later and tally up the reasons. They want to add a custom drop down list of reasons that they can select from. Is there any way to achieve this? Any kind of workaround if this cannot be done?

There is not a way to add an extra field for a completion/failure status. As comments is not wanting to be used, you could instead create more Completion Statuses that do not provide credit and use the Description field.
