Form hyperlink on Annual performance forms

Hi team,

We have created for our colleagues a Performance Check in form that stays open the entire financial year, it is being used by them and their managers to add comments related to their performance.

We would be looking to add a hyperlink to that performance check in on our Annual Performance forms that would allow users to easily access it. From our testing we were not able to do so, the hyperlink we add only takes users to the Forms inbox. Is anyone aware if there is a possibility to create a hyperlink that would allow users to open their Check in form?

Or are there similar requirements into your company? How did you achieve them? Do you keep the Annual performance form open throughout the year and allow users to populate their comments there directly instead of doing this onto a separate form?
